Student Enrollment Agreement    

By signing below, I agree to hold harmless Gracie Jiu Jitsu Barstow, GJJB, and its owners, managers, instructors, or coaches, responsible for any injuries and or losses that may occur during the martial arts program I am allowing my child to participate in. I hereby release, waive, discharge and covenant not to sue the facility used by my child, Gracie Jiu Jitsu Barstow, GJJB and or it’s owners, managers, instructors, or coaches. I fully understand that my child is participating in this martial arts program at his/her own risk with my permission and approval. I confirm that my child is in good health to participate in this program and there are no pre-existing injuries, medical conditions, disabilities, or impairments that will prevent my child from participating in this martial arts/karate program. I give permission to Gracie Jiu Jitsu Barstow,
GJJB to use photos and video footage of my child for promotional and marketing purposes. I understand that GJJB owns the rights to all photos and video footage and no compensation will be made for their use.

I have read and understood all the terms and conditions set forth herein and understand that any electronic signatures appearing on this agreement are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility.
